Write what was given, when

Medicine name, dose, and time prevent accidental double-dosing on a night shift. Temperatures with a time stamp help you brief NHS 111 or a GP accurately.

Partners need the same truth

If you share care, sync or show the health log before you hand over. “I think they had Calpol” is how gaps appear.

Logging isn’t diagnosing

Use the record to support a conversation with your care team. Dosing and “should we go in?” decisions belong with a midwife, health visitor, GP, or NHS 111 — not an app.
